Memberships are provided exclusively to uninsured patients of our practice and are considered pre-payment for preventive services or payment for access to discounted services. It is solely the patient’s responsibility to schedule and keep their appointments. No refunds will be issued under any circumstances, including failure to schedule and maintain appointments. Missed appointments or appointments canceled with less than 24 hour notice will incur a $25 missed appointment fee to reschedule per our appointment policy.
